2/28/19 Based on a information provided to the licensing office from the facility, the licensing inspector and another agency opened an investigation concerning care of children. Standard #: 22VAC40-185-410-1 Description: Based on the review of a videotape of the incident and staff interviews, the facility failed to ensure that staff do not use forbidden actions when disciplining children in care. Evidence: 1. Staff #1 roughly handled a child in the play room on 2/21/19 at approximately 3:05pm. Plan of Correction: I have spoken with all staff regarding discipline procedure.
