Comments:
The licensing inspector conducted an unannounced monitoring inspection on Wednesday, August 7, 2019 between 10:20 am to 2:00 pm to determine the center's compliance with licensing standards. During this inspection, there were 39 children in care with 10 staff including administrative staff. The licensing inspector observed the children in care participating in a variety of activities to include engaged in play on carpet, sitting in age appropriate equipment, working on coloring activities, participating in movie day "Ugly Dolls" in the classroom, completing bathroom breaks, interacting with peers and staff, transitioning into lunch time, and eating lunch. The children's lunch for today consisted of hamburger, kidney beans, oyster crackers, mixed fruit/pineapples, and milk. The school aged children group were observed at the center participating in a variety of activities to include working on puzzles with peers, playing games to include Connect Four and chess. Reportedly, there were some school aged children offsite at a field trip. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were reviewed by the inspector. During this inspection, there were six children's records and five staff records reviewed. The center's classrooms, bathrooms, diapering stations, indoor equipment were inspected. Standard #: 22VAC40-185-60-A
Description: Based on review of children' s record, the center did not ensure to obtain all required information. Evidence: In Child#3's record (start date 5/17/18), the second emergency contact information was missing with a notice there is no other emergency contact.

Plan of Correction: The center will contact parent today to get second emergency contact information.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview with staff, the center did not ensure diapering surface shall be used only for diapering or cleaning children. Evidence: 1. During the inspection on August 7, 2018, there was a box of tissue, tape dispenser, diapers, and wipes stored on the diapering station. Interview with staff confirmed the diapering surface is being used by children in care.

Plan of Correction: The center will give staff extra training to ensure diapering station is only used for diapering.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of staff records and interview with center's staff, the center did not ensure to obtain a copy of the results of a search for an out of state central registry. Evidence: In Staff#1's record (date of hire:7/17/19), the sworn statement document dated 7/17/19 indicated living out of state within last five years and no copy of results were found. Interview with staff confirmed that a request to obtain a copy of out of state central registry had not been completed during the inspection.

Plan of Correction: The center had staff complete background check information during the inspection and document will be notarized and mailed out tomorrow.
