An unannounced mandated Monitoring inspection was conducted today, 8/5/2019. There were 41 children present with 10 staff. Children were observed participating in water play, playing on the playground, diaper changing, reading books, drawing, and building with blocks. Reviewed 4 staff files, 4 children's files, medications and authorizations, emergency evacuation log, attendance, fire inspection, and physical plant. Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: Child#4 (start date 8/14/17) did not have documentation of immunizations prior to attending the center.

Plan of Correction: As with proof of physical, immunization record will be brought in ASAP. Parents were notified 8/5

Description: Based on record review, each child did not have a physical examination by or under the direction of a physician within 30 days of their first day of care. Evidence: Child#4 (start date 8/14/17) did not have documentation of a physical exam within 30 days of their first day of care.

Plan of Correction: PArents have been notified. Will bring in paperwork ASAP when they return from vacation.

Description: Based on staff record review, documentation that two or more references as to character and reputation, as well as competency, were not checked before employment. Evidence: Staff#2 (start date 12/11/18) did not have documentation that two references were checked prior to employment.

Description: Based on observation, cosmetics, medications, or other harmful agents were being stored in areas and purses that were accessible to children. Evidence: Purses with potentially hazardous items were observed being stored accessible to children in the Lions and Little Monsters classroom.

Plan of Correction: Notes were shared with both rooms and the violations were rectified on the spot

Description: Based on observation, electrical outlets did not have protective covers. Evidence: An accessible surge protector in the Lion's room did not have a protective cover.

Description: Based on observation, the center is administering prescription medication that is not maintained in the original, pharmacy labeled container. Evidence: Observed Child #5 (Avi-Q) and Child#6 (inhaler) being maintained in a plastic bags and not in the original pharmacy labeled container.

Description: Based on staff record review, the center did not obtain the results of a search of the central registry by any other state in which the individual has resided in the preceding five years within 30 days.
