Comments:
An unannounced monitoring inspection was conducted beginning at 10:45 a.m. and ended at 2:30 p.m. with the director and assistant director. There were 44 children in care ranging in age from 3 months to 11 years old under the care and supervision of 10 staff. Infants were observed sleeping, having playtime on the floor and being fed. The green and yellow rooms (toddlers, two year olds) were observed on the playground. The red class (three year olds) were observed during story time. The blue class (fours/five year olds) were observed working on a project and practicing sharing. Eight children's records and five staff records including one new staff member were reviewed. Emergency drills were up to date. The monthly menus are posted. The toddler playground was inspected. The large playground is currently marked off limits due to some maintenance needs and was not inspected during today's inspection. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in safe, operable condition. Evidence: On the toddler playground, there are two Tonka brand trucks that have the paint chipping that creates a scratching hazard on the dump truck beds and are rusted. There is a Little Tykes ride-in car with broken plastic eyes that are rough and create a scratching hazard.

Plan of Correction: Removed during the inspection.

Description: Based on observation, not all diapering pads were not nonabsorbent. Evidence: In the Green Room, there is an inch long tear in the side of the nonabsorbent cover of the diapering pad.

Plan of Correction: A new diaper pad will be purchased today.

Standard #: 22VAC40-185-520-C
Description: Based on observation, not all diaper ointment was labeled and kept out of reach of children. Evidence: There was an Aquaphor Healing Ointment that was not labeled with the child's name and it was on the floor in the bathroom shared by the Yellow and Green rooms.

Plan of Correction: Ointments will be labeled and put out of reach of children.
