Technical Assistance:
Technical assistance today included licensing standards regarding records, required postings, emergency procedures and physical environment. The fire inspection expired 8/1/2019, but an appointment is pending. New director is scheduled for training at DSS in September.

Comments:
An unannounced monitoring inspection was conducted today from 11:00 - 2:15 pm. There were 57 children present in both buildings. All classes were within ratio. I observed waterplay outdoors, listening to a story; eating lunch and preparing for naptime. I enjoyed listening to a class deciding whether it was appropriate for a princess to wear hiking boots!. Standard #: 22VAC40-185-50-A
Description: Based on observation during today's inspection, not all records were locked as required. Evidence: Each class had a binder with emergency contacts and other information that was not kept locked.

Plan of Correction: We will make changes to make sure emergency information is kept confidential.

Standard #: 22VAC40-185-70-A
Description: Based on observation during today's inspection, items from staff records such as emergency contacts were not available. Evidence: The information was in a locked office, and no staff had a key this morning.

Plan of Correction: This will be worked on.

Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, some areas of the center were not maintained in a safe and operable condition. Evidence: Indoors: There were areas of peeling and chipped paint throughout the first building (house). There were cracked electrical switchplates observed at child height in both buildings. Dangling cords from phones and radios were accessible. Outside -on the stairways, some treads were torn, causing a tripping hazard. There were three protruding nails on the wooden play structure and some sharp areas of wood on the play house. There were five rocks missing in the ground of the "Stepping Stone Path", leaving holes that are tripping hazards and a place for mosquitoes to breed. A small trampolene with small holes in it was on the playground.

Plan of Correction: This items will be fixed.

Description: Based on observation during today's inspection, there were hazardous substances in unlocked areas. Evidence: On the window sill of the kitchen above the sink, Steramine tablets were observed today. A supply closet off one room was not locked, and a supply of bleach was stored inside. Clorox wipes were observed on the window sill of the children's bathroom.

Plan of Correction: Staff will be retrained.i

Standard #: 22VAC40-185-330-B
Description: Based on observation during today's inspection, there was not an adequate amount of resilient surface spread in fall zones as required. Evidence: While some areas had a large amount of pea gravels, the areas under and around fall zones did not have enough resiliency.

Plan of Correction: This will be done.

Standard #: 22VAC40-185-510-A
Description: Based on a review of records today, one child's authorization for medication was no longer effective. Evidence: The authorization for the allergy medication for Child #1 expired 8/8/2019.

Plan of Correction: It will be given to the parent today.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of records today, not all results of fingerprint search had been received, and there was no documentation of it being checked. Evidence: Staff # 1 began work on 6/10/2019, but no results from a fingerprint search had been documented.

Plan of Correction: A copy will be e-mailed.
