Standard #: 22VAC40-185-130-A Description: Based on the review of five children's records and verification from the director, the center failed to obtain documentation that one child had been adequately immunized before the first day of attendance. Evidence: 1. The first date of attendance for Child #4 was 03/26/2018 and the provider did not obtain documentation of the child's immunizations until 04/19/2018. 2. Staff Member #1 confirmed the record of immunization had not been received before the child's first date of attendance. Plan of Correction: The provider will ensure that moving forward an immunization record showing a child has been adequately immunized will be obtained before a child attends the center.
Standard #: 22VAC40-185-140-A Description: Based on the review of five children's records and confirmation from the director, the center failed to obtain documentation of one child's physical examination within one month of the child's first day of attendance. Evidence: 1. The first date of attendance for Child #2 was 02/06/2019 and the provider did not obtain documentation of the child's physical examination until 03/25/2019. 2. Staff Member #1 confirmed the physical examination report was received over a month after the child's first date of attendance. Plan of Correction: The provider will ensure that moving forward a copy of a child's most recent physical examination will be obtained before a child attends the center.
