Comments:
An unannounced monitoring inspection was conducted on 08/20/2019 from 9:13am - 1:00pm. At the time of the tour there were 62 school age children in care with six staff. Children were observed playing a basketball game in the gym. Arts & craft activities observed in the I-Zone classroom. A field trip departure was reviewed. Lunch was also observed. Records reviewed for eight children and six staff. Medication and emergency supplies additionally reviewed. The violation is listed on the violation notice and were discussed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe condition. 1. The following unsafe conditions were observed in the outdoor play area: a. A tree root, measuring approximately 12 inches in length, was observed along the ground of the entry area to the tents from the sidewalk. This is a trip hazard. b. A broken corn hole board was observed on the ground behind the tent area. The decayed wood was sharp and splintered in many areas. c. A splintered and broken board was observed on the ground in the middle of the tent area. This board contained splinters and sharp areas. d. A circular hole, containing a small amount of water, was observed in the middle of the tent area. This hole was covered up by a broken board. This is a trip hazard. e. A metal support bar for the tent had come loose from the tent was was sticking up out of the ground approximately 6 inches. This is a trip hazard. f. Nine tree stumps were observed on the ground surrounding the covered tent area. These stumps were in various stages of deterioration with sharp edges and splinters. 2. The following safety hazards were observed in the I-Zone classroom: a. A cord was dangling down approximately 50 inches. This is an entanglement hazard. b. The wall mounted television and DVD player were plugged into an extension cord which was plugged into a wall outlet. This is a fire safety hazard.

Plan of Correction: Site Director met with property team and all corrections have been fixed as of 8/20/19 and 8/21/19.
