Technical assistance today included physical environment.

Comments:
An unannounced monitoring inspection took place today from 1 - 3:15 pm. There were 81 children present today. I observed schoolagers coloring and watching a video after returning from a field trip; and observed naptime for other classes. All rooms were within ratio. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's files today, not all required information was present. Evidence: Child # 1 had no emergency contacts listed. Other files did not have documentation that emergency information had been updated annually by parents.

Plan of Correction: We are starting a computerized updating next week.

Standard #: 22VAC40-185-270-A
Description: Based on observation during today's inspection, some areas and equipment of the center were observed to not be maintained in a safe and operable condition. Evidence: Outdoors: 1. A very large area (over 35 feet) over bare dirt was covered by exposed, ripped landscape netting. The entire area represents a tripping hazard. This has been cited previously on 4/23/19 and 4/26/18. 2. A small wooden picnic was wobbly, with loose, protruding bolts. 3. A tricycle was missing a handlebar cover. 4. There was a badly torn screen on a window. 5. The cart carrying balls was very wobbly and could tip over. 6. The dryer vent was not covered with anything protective today, leaving a sharp edged hole at child height. 7. Some debris on the playground today included a coin-sized floral bauble with looked like a candy. It was on the slide. Indoors: 1. There was child's cot blocking the exit door in one room at naptime. 2. A toilet was broken and could not be used. 3. Areas of chipping paint were observed in two rooms. 4. The taped contact paper on the side of the shelves in the infant room was coming off.

Standard #: 22VAC40-185-330-B
Description: Based on observation today, there were surfaces on the play area not covered with an adequate amount of resilient material. Evidence: Although there were stacks of mulch piled against certain areas of the slide, there were some areas that had not been swept to maintain the required amount of resilient material.
