Technical Assistance:
Consultation was given on cot spacing to ensure 12 inches between occupied cots.

Comments:
An unannounced monitoring visit was conducted today. Forty seven children were present in 5 classrooms with direct care staff in ratio. Observed transition from lunch to hand washing, rest time, infant care and story time in the Grizzly classroom. Lunch was bought from home today. A sample of staff records were reviewed, emergency medications and storage, emergency preparation and care of children was observed. Program will be closed for in-service training the last 2 days of August for staff development and room set-up. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A
Description: Based on record review, 1 of 3 staff records is incomplete. Evidence: Staff #2's record is missing documentation of 2 references checked prior to hire ( doh 5/28/2019)

Plan of Correction: Reference documentation will be obtained and placed in staff record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center are not being maintained in a clean and operable condition. Evidence: 1. The cabinet knob on a cabinet in the Panda classroom is broken and a screw is exposed. 2. The tiles throughout the center are stained. 3. Paint is peeling on the window sill in the Panda classroom and in the Grizzly Room door frame 4. A broken wire is hanging off the roof of the shed on the playground 5. A drum is cracked on the playground and split

Plan of Correction: The floors are being stripped and rewaxed over Labor Day. The physical coordinator will be contacted to paint peeling paint and fix the two items on the playground. Tape will be placed over the screw until the cabinet knob can be replaced.

Standard #: 22VAC40-185-290-3
Description: Based on observation, all electrical outlets did not have protective covers. Evidence: Two outlets in the Panda classroom and two outlets in the infant classroom were not covered by protective covers.

Plan of Correction: Outlet covers will be replaced by the end of the day.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff statement, a hands free leakproof covered receptacle for soiled diapers is not available. Evidence: The trash can in the infant room for soiled diapers was broken and would not operate hands free.

Plan of Correction: The can will be replaced by Friday.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, the center did not follow procedures for medication. Evidence: 1. Medication was expired for 2 children: Medication for child #1 expired 7/2018 and was not returned within 14 days of the authorization expiration. Medication for Child #3 expired 7/2019. 2. Written authorizations for the administration of medication were expired or incomplete: Child #3's authorization expired 5/2019. A medication authorization for Child #1's medication was not on-site.

Plan of Correction: Medication will be returned to the parent and an e-mail will be sent to the parent to complete a new authorization form.

Standard #: 22VAC40-185-550-E
Description: Based on staff statement and log, two emergency drills were not recorded. Evidence: Documentation of drills for the months of June and July 2019 were not written down.

Plan of Correction: Center management will record dates to complete the log.
