Discussion of the subsidy regulations and how they apply to the Child Day Center (CDC). Discussion about the current allergy list and allergy plan for the one applicable child. To avoid cross contamination, food is brought from home. All other children utilize the catering services for meals.
A Subsidy Health and Safety (SHSI) inspection was conducted at the Building Blocks II Day School located in Falls Church, Virginia. This inspection was conducted on 08/27/2019, between the hours of 8 am through 11 am to review supplemental health and safety requirements. An Unannounced Monitoring Inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that were not covered in licensing standards were reviewed during this supplemental inspection. Standard #: 22VAC40-665-580-B Description: Based on review, 1 of 12 staff files indicated that a certificate of the Virginia Preservice training within the first 90 days of employment was not available. Evidence - 1. On the date of inspection (08/27/2019), a certificate of completion issued by Penn State for the Virginia Preservice Training for Child Care Staff was not available for review for Staff #8. 2. The start date for Staff #8 was listed as - 08/30/2018. 3. It is unclear if Staff #8 has completed this training at all. Plan of Correction: A copy of this certificate shall be obtained and placed on file, if applicable, Staff #8 shall complete these 10 hours of training.
