A complaint investigation was begun at the facility on 7/23/2019 in response to a complaint received in the licensing office on 7/17/2019. The investigation continued on 8/12/2019. The investigation included review of children and staff records, review of injury records and staff interviews. The complainant alleged that children were being injured while in care and that injuries were either not being observed and/or being reported to parents. The investigation revealed the allegation to be valid and a violation was cited which can be found on the violation notice.

Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based upon staff interviews and review of written injury records, the facility has not maintained a written record of children's injuries in which entries are made the same day of occurrence that includes all required information.
Evidence:
1. Staff interviewed revealed that child 1, age 13 months, had a bite mark on her cheek. There was no written record of this child's injury or of the parent being notified of the injury.
a. Staff 1 stated that she had spoken with the parent of child 1 regarding the bite mark but that no staff had witnessed the injury occurring. Staff 1 stated that they did not complete a written record of the injury as no staff saw it happen.
b. Three staff interviewed indicated that there are currently two children in the infant/young toddler grouping (child 2 and child 3) who have been biting.
c. There was no written record in the injury log of any children being bitten while in care.
4. There was one written record in the injury log. The record was for child 2 and was dated 8/8/2019. The written record did not include the date and time the parent was notified of the injury or the method by which the parent was notified (telephone, in person, etc.).Plan of Correction: Not available online.
