Technical Assistance:
Discussed policies and procedures regarding sanitizing toilet training seats.

Comments:
An unannounced monitoring inspection was conducted from approximately 9:00 am-1:15pm. At the time of entrance, 42 children were in care with 7 staff members present. Ratios were in compliance. Children were observed during snack time, engaged in group activities, circle time, exercising, small motor activities, manipulatives and engaged with staff. Interaction were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. A selection of 3 staff and 4 children records were reviewed. The physical space, evacuation drills, attendance records, 5 injury reports, a sampling of medications and required posted documents were also reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-60-A-7
Description: Based on record review, an allergy action plan was unavailable for child #2. Evidence: The physical examination signed by the physician, stated child #2 had an allergy and required an epinephrine injector as treatment. There was no allergy action plan or medication on site for this child. Staff #1 stated they were told by the parents that child #2 had no allergies.

Plan of Correction: Called the parents (while the licensing inspector was here) and they said they will bring a letter from the pediatrician stating that the child outgrew the allergy and does not need medication anymore.

Standard #: 22VAC40-185-510-E
Description: Based on observation and interviews, medication was not labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: The required label for child #1's medication was not available for review. The medication was in a container labeled with the child's name on the outside and the authorization form on the inside.

Plan of Correction: Called the parents (while the licensing inspector was here) and they are sending me labeled medication by the end of this week.

Standard #: 22VAC40-185-510-N
Description: Based on record review, the authorization for the medication on site had expired. Evidence: The authorization for the medication, for child#1, expired on 04/17/19.

Plan of Correction: Called the parents (while the licensing inspector was here) and they are sending me an updated authorization by the end of this week.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 out of 3 staff records, had an incomplete Central Registry finding. Evidence: Staff #2 (date of hire 11/16/16) did not have a Central Registry finding available for review. A copy of the Central Registry document was in the employee's folder but the official copy with the results could not be found.

Plan of Correction: We will call and resubmit if needed.
