Child records A, B, C, and F do not include documentation of a shot record. Each of these children have already entered care.Plan of Correction: I will ask the school nurse for a copy of each child's health records.
Standard #: 22VAC40-185-510-C Description: Based on a review of children's medications, it was determined that one long term authorization was not fully completed.
Evidence:
The authorization for one child's long term non-prescription medication does not include a clear documented start/stop date for which the authorization is effective.Plan of Correction: I will ask the parent to obtain the missing information.
