Comments:
An unannounced monitoring inspection was conducted from 8:40 a.m. and ending at 1:00 p.m. with the center director and assistant director. There were 64 children in care ranging in age from 5 months to 12 years of age with 9 staff. Infants were observed having play time and being fed. Toddlers were having free play. Two year olds were observed during transition, diapering and coloring. The Montessori classroom was observed during circle time learning about shapes. JK (four and five year olds) were observed during centers that included a variety of activities and manipulatives. Three year olds were observed on the playground.School agers were observed during outdoor time that included playing on the playground and a game of hot potato. School agers were observed during transition inside. The playground was inspected. Seven children's records and five staff records were reviewed. Nine medications, authorizations and the administration log were reviewed. Emergency drills were reviewed and up to date. Please provide a plan of correction and date to be corrected by no later than close of business 8/27/2019. Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of center were maintained in a safe, operable condition. Evidence: In the Stepping Stones (two year old classroom), the window at the back of the classroom the windowsill has two holes in the wood with chipped, peeling paint and deterioration of the wood itself. The second hole is approximately the size of a nickel and has jagged edges that can be a scratching hazard. The gate to the school-age playground is broken and is disconnected from the top hinge and poses a crushing hazard.

Plan of Correction: Maintenance came out on 8/27/2019 and fixed the windowsill. The gate on the playground was fixed on 8/23/2019

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning and sanitizing materials were stored beside utensils and single-service articles. Evidence: In the Montessori classroom, there were two bottles of cleaner and one bottle of sanitizer stored beside single-use plastic cups, plasticware and plates.

Plan of Correction: The cleaning and sanitizing materials were stored in a locked cabinet out of reach. Plates, cuts and plasticware were moved to another cabinet during the inspection

Standard #: 22VAC40-185-510-A
Description: Based on review of documentation, the center did not obtain written authorization from the parent to administer medications. Evidence: A medication administration log for Child #7 indicates administration of benadryl on 7/11/2019. There was no written authorization from the parent to administer benadryl.

Plan of Correction: correct forms were also given to the parent on 8/22/2019

Standard #: 22VAC40-185-510-C
Description: Based on review of documentation, the center did not follow its procedures for medication. Evidence: There was no long-term written authorization from the child's physician and parent for an Epipen for Child #8 and written authorization from the child's physician for benadryl and Auvi-Q for Child #7.

Plan of Correction: FARE form is on file for the child with parent and physician signature for authorization. Long term authorization was given to the family on 8/22/2019 for signatures.
