An unannounced monitoring inspection was conducted from 4:00 -5:30 pm with staff. There were 33 school age children upon arrival, with four staff supervising. The children were observed completing homework, washing hands and eating snack, and participating in a variety of structured center activities. Snack served today was bagels with cream cheese and grapes. Required information was posted. There were no medications to review though two staff members are Medication Administration Training (MAT) certified. Five children's files were reviewed and in compliance. The last fire drill was conducted: 8/29/19 and the last shelter-in-place drill was conducted: 8/30/19. Standard #: 22VAC40-185-80-A Description: Based on a review of the written attendance record and staff interview(s), it was determined that the facility did not ensure all children were included on the written attendance record. Evidence: The attendance record listed 23 children in attendance, however the LI observed 33 children in attendance. Plan of Correction: Will ensure the attendance sheet is updated when children arrive/depart.
Standard #: 22VAC40-185-340-A Description: Based on observation, the facility failed to ensure the children's care, protection and guidance. Evidence: on 9/4/2019, the staff did not have an accurate count of the number of children in care. Before leaving the cafeteria to go wash the children's hands, staff did count the children, however before coming back into the cafeteria from the bathrooms, staff did not count the children. When asked by the licensing inspector, one staff stated there were 30 children present, another stated there were 37 children and a third staff member didn't know and had to start counting. The Licensing Inspector observed 29 children at the time. Plan of Correction: Will ensure procedures for identifying where children are at all times are followed.
