Technical Assistance:
Technical assistance was provided regarding out of state background checks, on the DSS website type in Adam Walsh in the search box for a list of the state information. A discussion was also held on the renewal process.

An unannounced renewal inspection was conducted today from 10:00am-2:00pm. There were 85 children (infants-5yrs.) supervised by 14 direct care staff. The physical plant, 8 staff records, 8 children?s records, medications, evacuation drills, injury reports, emergency supplies,programming and policies were inspected. Children were observed participating in group play, laying outside, reading books, playing outside, eating lunch and nap time. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Description: Based on review of 8 staff records, do not have documentation of an updated tuberculosis (TB) screening every 2 years.
The following staff have expired TB screenings on record, and do not have documentation of an updated screening.

Plan of Correction: Staff #2 Current TB test was in the ?to be filed? folder and has now been placed in employee?s file. Employee in compliance

Description: Based on review of 8 staff records, 1 does not have documentation of qualifications for the position.
Staff #1 is in a leadership role, with no documentation of qualifications for the position.

Plan of Correction: Qualifications for Staff #1 were verified upon hire; will get another copy of credentials/degree for her file

Standard #: 22VAC40-185-210-A
Description: Based on review of 8 staff records, 2 do not have documentation of completion of a high school program or the equivalent.
Staff #1,and 3 are in a Lead role and do not have documentation of completing a high school program or equivalent.

Plan of Correction: I will get the required documentation and put it in their record. Going forward I will get the documentation during the hiring process.

Standard #: 22VAC40-185-280-B
Description: Based on observation potentially hazardous substances were accessible to children.
Located in the Language Arts classrooms, 2 hand lotions were accessible to children, both bottles were labeled "keep out of reach of children".

Plan of Correction: I've already removed the lotions.

Standard #: 22VAC40-185-550-M
Description: Based on review of injury reports, not all reports are complete.
Injury reports do not include how and when parents were notified of injury.

Plan of Correction: I will retrain the staff and management will not sign if all information is not recorded.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of 8 staff records, 1 record does not documentation of an out of state background check from a state they have lived in within the past 5 years.
Staff # 6 has lived out of state within the past 5 years and does not have documentation of a background check from that state.

Plan of Correction: I will send it the required form for that state. In the future I will send out the forms during the hiring process.
