Comments:
The licensing inspector conducted an unannounced renewal inspection Friday, August 30, 2019 from 10:05 am to 1:02 pm. The infants were observed playing on the floor and sleeping. The older infants and toddlers were observed eating lunch. All classrooms and the playground were inspected today.
Five children?s records, five employee records, and three board officer records were reviewed during this inspection.
The center administers medication, however none is being administered at this time.
The center's first aid kit and emergency supplies were inspected.

Today, the following child to staff ratios were observed:
Infant I (6 weeks to 12 months) ?8:3
Infant II (12-14 months) ?3:2

Toddler II (18-24 months)- 4:2
School Age ?not present

The violations from the previous inspections were checked for corrections. There were two repeat violations found during today?s inspection.

Description: Based on a review of children's records on 08/30/2019, the center did not ensure that documentation of additional immunizations was obtained once every six months for children under the age of two years.


EVIDENCE:

The record of Child #1, who is 13 months old, and has been enrolled at the center since 09/04/2018, did not contain documentation of immunizations every six months.

Description: Based on a review of children's records on 08/30/2019, the center did not ensure that each child's record reviewed contained the required information.

EVIDENCE:

1. The record of Child #5 did not contain information regarding chronic physical problems and pertinent developmental information and any special accommodations needed.
2. The record of Child #5 did not contain information regarding previous child day care and schools attended by the child.
