Technical Assistance:
The licensing inspector and administration reviewed the use of medication authorization forms and topical skin product forms. They also reviewed standard 22VAC40-185-500-B-3 to ensure that a nonabsorbent surface shall be used for diapering or changing.

Comments:
An unannounced monitoring inspection was conducted on August 26, 2019 from approximately 9:15 AM to 1:50 PM to determine the facility's compliance with licensing standards. The staff to child ratios were observed as follows: Infants 1:4,Toddlers 1:5, 2 year olds 1:8, 3 and 4 year olds 2:13, Kindergarten 1:10, School Age I 1:18, School Age II 1:20. Lunch today consisted of macaroni and cheese, carrots, applesauce, and milk. All areas used for child care were inspected to include bathrooms, hallways, kitchen, playground, buses and classrooms. The facility has age appropriate materials and supplies. The following required information was posted; the License, most recent inspection notice, emergency numbers, evacuation map/procedures, daily schedules, menu, and various parent information. The first aid kits and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Seven staff records and seven children's records were reviewed. Medications and authorizations were reviewed. There are staff on site with current First Aid, CPR, MAT and DHO training. The most recent evacuation drill was documented on 8/19/19. The most recent shelter-in-place drill was documented on 2/25/19.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure that all areas shall be maintained in a clean, safe and operable condition. Evidence: 1. The licensing inspector observed four protruding screws located on the fencing of the toddler playground. 2. The Kindergarten classroom had two areas of peeling paint on the walls.

Plan of Correction: Per administration, the nails were removed immediately and the wall was repaired the same day.

Standard #: 22VAC40-185-550-M
Description: Based on a review of emergency documentation, the center did not ensure that injury records contained the required information. Evidence: The licensing inspector observed that four of the seven injury records reviewed did not contain documentation showing how parents were notified of the injury; or the date and time parents were notified of the injury.

Plan of Correction: Per administration, the incident report was updated upon review on 8/26/19.

Standard #: 22VAC40-191-40-D-6
Description: Based on a review of seven staff records, the facility did not obtain a fingerprint-based criminal history record report from one staff that is dated no more than 90 days prior to the date of employment. Evidence: The licensing inspector observed the record for Staff #4 (hire date 7/8/19) contained a fingerprint-based criminal history record report dated 9/4/18.

Plan of Correction: Per administration, staff completed field print on 9/4/19 within 10 days as required.
