Comments:
The licensing inspector conducted an unannounced inspection on Friday, September 06, 2019, in response to a self-reported incident that the Department received on August 21, 2019 regarding serious injury reporting. The inspector was on site at the center on September 6, 2019 from 4:06pm to 5:01pm. An interview with staff was completed and documentation was reviewed. The preponderance of the evidence gathered during the investigation validates the self-reported incident of late serious injury reporting. Violations were cited regarding serious injury reporting. Violations:
Standard #: 22VAC40-665-550-2
Description: Based on record review and an interview, the vendor did not ensure to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence:
1. The center's representative made contact with the department regarding the serious injury of child #1 on 8/21/2019, the injury occurred on 8/13/19. All vendors are required to contact the department of any serious injuries no more that two business days after the injury occurs.
2. The center's representative made contact with the department regarding the serious injury of a child on 8/21/2019, the injury occurred on 8/16/19. All vendors are required to contact the department of any serious injuries no more that two business days after the injury occurs.

Plan of Correction: 1. Staff will communicate to CPS about a child needing treatment from a medical professional once the incident form is completed and communication with parents are done. This will be done within 2 business days.
2. Staff will be communicated with during trainings to understand the procedure that is to be taken when regarding serious injury of a child.
3. The Childcare Director along with their staff and leadership team will be responsible for implementing each step and monitoring any preventive measure.
