Comments:
An unannounced monitoring inspection was conducted today between 9:00 am and 12:30 pm. There were 43 children present with 12 staff members. The children were observed playing with floor toys, hand washing, eating snack and during a fire drill. The physical plant, children's records, staff records, evacuation drill log, attendance logs, emergency plans, snack menu's and emergency supplies and medication's were inspected. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of childrens' records, the center failed to ensure that children's records are up to date.

Evidence:
Child #3's record did not include a phone number for the second emergency contact.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, the center failed to ensure that staff records are complete and up to date.

Evidence:
1) Staff #2's record did not contain documentation of the emergency contacts address.
2) Staff #5's record did not have documentation of two completed reference checks.or documentation of orientation.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method.

Evidence:
1) In the 2's classroom, the bleach/water solution was not in a locked place.
2) In the 4's classroom, bleach wipes and a bleach/water solution were not in a locked place.

Plan of Correction: We will find a safe way to lock these items

Standard #: 22VAC40-185-550-D
Description: Based on a review of evacuation log, the center failed to ensure that a monthly practice fire drill and a minimum of two shelter in place drills were conducted annually.

Evidence:
The center only had documentation of one shelter in place drill.

Plan of Correction: We will conduct all required drills.
