63.2 Child Abuse & Neglect

Comments:
An unannounced monitoring inspection was conducted today from 9:00 am-12:30pm. There were 73 children (infants-5yrs.) supervised by 16 direct care staff. The physical plant, 4 staff records, 6 children?s records, medications, evacuation drills, injury reports were inspected. Children were observed participating in group play, story time, and circle time. There books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The playground and surfacing was inspected. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on review of children records, the center did not maintain completed documentation of viewing proof of the child's identity and age Evidence: 1. Incomplete documentation for Child #1 (started on 6/17/19) did not include staff signature and date of birth certificate was reviewed.

Plan of Correction: Moving forward will verify and date all children files.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not not maintain areas and equipment of the center, inside and outside, in a clean, safe and operable condition. Evidence: 1. The boys bathroom located in the hallway had rust on the bathroom stall. 2.. The fence line on the playground is leaning inward, posing as a safety hazard. The fence is bending onto the playground. 3. A piece of the hallway panel located outside the 4's classroom was sticking out, posing entanglement clothing or snag skin.

Plan of Correction: Will let the owner know the concern of bathroom stall. Will call to request repair to the fence. Hallway will be fixed this evening,

Description: Based on observation, potentially hazardous substances such as cleaning materials were stored in a area that was not accessible to children. Evidence: In the boys bathroom located in the hallway, disinfected spray was on a toilet, accessible to children.

Plan of Correction: Note reminder to staff not leave hazardous items in the stall. Corrected during inspection
