Comments:
An unannounced Renewal inspection was conducted from 11:00am to 1:45pm. There were 44 children in the direct care and supervision of 10 staff. A sample of 6 children's records, 4 staff records, 5 emergency medications, the first aid kit, evacuation drills, emergency materials, the playground, and the physical plant were reviewed. Children were observed playing on the playground, doing arts and crafts, listening to music, and coloring. If you have any questions, please contact me at shahana.green@dss.virginia.gov . Thank you.

Violations:
Standard #: 22VAC40-185-240-C
Description: Based on a record review, the center did not ensure that all required annual training hours (16 hours per staff member) were obtained for all staff.
Evidence:
1. Staff # 2 only had 8 hours of professional development documented within the past year.
2. Staff #3 only had 10 hours of professional development documented within the past year.
3. Staff #4 only had 12 hours of professional development documented within the past year.

Plan of Correction: For the training hours for the staff, we will make a training log to make sure that each staff member has their 16 hours annually.

Standard #: 22VAC40-185-510-F
Description: Based on a review of medications, 1 out of 5 medications was not in the original container.
Evidence:
1. Emergency Medication for Child #5 was missing the original container.

Plan of Correction: We will ask the parents to bring in the box.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medications, medication with expired authorization was not returned to the parent or disposed of by the center within 14 days.
Evidence:
1. Child #6 had an emergency medication on-site with an authorization form that expired on 1.25.2019.

Plan of Correction: We will ask the parents for the new authorization form.
