Comments:
An unannounced Monitoring inspection was conducted on September 16, 2019. There were 20 children present in two classrooms. Four staff were supervising the children's activities.
A sample of 5 children's records was reviewed. Complete staff records were reviewed at the administration office on 8/26/2019 for 5 staff.
The inspection was conducted from 8:42 AM to 11 AM.

Discussion with site director about standard 22 VAC 40-185 -70 A-1 for staff documentation that is required to be at the center.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center failed to ensure that all physical examinations were obtain within 30 days of enrollment.

Evidence;

1. Child # 3 date of enrollment was 2/12/2019 and the date on the physical exam was 8/1/2019.

Plan of Correction: The FSW will in the future obtain the physical exams within the 30 days required.
