An unannounced Monitoring Inspection was conducted on September 18, 2019 from approximately 8:15 am until 1:30 pm. Upon entrance there were 77 children and 11 staff present with children. Additional staff were on site for program and administrative purposes. The record review sample consisted of 8 children's records and 6 staff records. Seven medications were reviewed. Children were observed playing outside, eating lunch and during free play with cars and trucks. The results of the inspection were reviewed and verified with the Center Administrator during the exit interview. Areas of noncompliance are noted on the violation notice. Standard #: 22VAC40-185-70-A Description: Based on review of 6 staff records, the licensee did not ensure that each staff record includes written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job position.
Evidence: The record for staff #2, listed as a program leader did not have documentation the individual possesses the education, staff development and certification required by the job position.Plan of Correction: The center administrator has spoken with staff and the obtain the documentation.
Standard #: 22VAC40-185-510-J Description: Based on observation and staff interviews, the licensee did not ensure that all medication was stored in a locked place using a safe locking method that prevents access by children.
Evidence: In a first aid kit stored in an unlocked closet, there were 5 packets of aspirin, 5 packets of non-aspirin and 5 packets of antacid. Total of 15 packets of medication were stored in an unlocked manner.Plan of Correction: The center administrator removed medication and stored it in a locked place.
Standard #: 22VAC40-185-540-E Description: Baaed on observation and staff interviews, the licences did not ensure there was one working, battery-operated radio in each building used by children and any camp location without a building.
Evidence: In the 2nd building, gym area there was not an operable battery operated radio.Plan of Correction: The Center Administrator will supply the radio and batteries to the building.
Standard #: 22VAC40-185-560-G Description: Based on observation and staff interviews, the licensee did not ensure the food containers shall be sealed and clearly dated and labeled in a way that identifies the owner.
Evidence: Three of 4 lunch boxes were reviewed and 3 were not labeled in way that identifies the owner and 1 did not include the date.Plan of Correction: The Center Administrator will ensure those are taken care of today.
