Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:45 am - 1:45 pm. There were thirty five children in care: seven infants with two staff, four toddlers with one staff, four two years old with one staff, six three years old in one class and seven three years old in the other class, and seven four years old with one staff. The director was also present. The children were observed playing, eating lunch, in group activities and preparing for nap time. Six staff's records and five children's records were reviewed. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record reviews, the center failed to obtain documentation of a physical exam.
Evidence:
1. The inspector reviewed five children's records. One of five records reviewed (child record #4) did not have documentation of a physical exam. Child's date of enrollment was 10/08/2018.

Plan of Correction: The director will contact the child's parents and request a copy of the child's physical.

Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records.
Evidence:
1. The inspector reviewed five children's records. One of five records reviewed (child record #5) did not contain proof of identity and age. Child's date of enrollment was 06/03/2019.

Plan of Correction: The director will contact the child's parents and remind them that this information is needed for the child's record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the facility in a safe and operable condition.
Evidence:
1. The inspector observed the outside play areas. Outside of the mulch barrier on the preschool playground, there are tree roots creating tripping hazards. There are also tree roots on the infant/toddler playground creating tripping hazards. The inspector also observed one inoperable toilet and a broken toilet paper holder in the day care bathroom.

Plan of Correction: The director said additional soil will be added to cover the roots on the play areas. A plumber is coming this weekend to repair the toilet. The director will order a new toilet paper holder today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to have all hazardous substances stored in a locked place.
Evidence:
1. The inspector observed the center. There was a container of disinfectant spray and a container of Febreze in a wall shelf in the girl's bathroom on the day care side. The containers were inaccessible to the children, but were not stored in a locked place. There was a container of disinfectant spray and a container of hair spray in a unlocked base cabinet in the girl's bathroom on the church side.

Plan of Correction: The director will placed the disinfectant spray and Febreze in the locked cabinet in the girl's bathroom on the day care side. The director will place a safety lock on the cabinet door in the girl's bathroom on the church side.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center failed to maintain resilient surfacing on the playground at the required depth.
Evidence:
1. The inspector observed the preschool playground. There was approximately 3 - 4 inches of mulch under and surrounding the climber. Fall height of the climber is 40 inches.

Plan of Correction: The center has additional mulch and it will be added to the play area this weekend.

Standard #: 22VAC40-185-510-G
Description: Based on observation, the center failed to have medication in it's original container.
Evidence:
1. The inspector observed the medication for child #2. The medication was not in the container with the prescription information attached.

Plan of Correction: The director will request the original container for the medication from the parent today.

Standard #: 22VAC40-185-510-J
Description: Based on observation, the center failed to have medication stored in a locked place.
Evidence:
1. The inspector observed medication in the two year old classroom. The medication was stored in a wall shelf and inaccessible to the children, but not in a locked place.

Plan of Correction: The director will place the medication in the locked box in the kitchen today.
