An announced initial inspection was conducted between the hours of 9:00am and 11:00am. A tour of the facility was conducted with the owner. Policies and records were also reviewed.

Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure all areas of the center, inside and outside were maintained in a safe condition.
Evidence:
1. There were 2 slats that measured over 4.5" on both sides of the stairs leading to the playground.
2. There was a large wooden giraffe that had an area of splintered wood and peeling paint.Plan of Correction: The following corrective action was provided by the owner during the exit interview:
Lattice will be placed along the stair slats to ensure there are no gaps. The wooden giraffe will be sanded and a clear paint gloss applied.
Standard #: 22VAC40-185-540-E Description: Based on observation the center did not ensure all required non-medical emergency supplies were available.
Evidence:
The center had an emergency radio, however did not have extra batteries.Plan of Correction: The following corrective action was provided during the exit interview:
The owner will purchase extra batteries to be kept at the center.
