Comments:
An unannounced monitoring inspection was conducted on September 9, 2019 from approximately 10:00 am until 11:45 am. Four staff were present with eleven children during the tour of the facility. Additional staff were on site for administrative duties. Five children and five staff records were reviewed.

Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. The providers responses for the ?Description of Action To Be Taken? were not received as of September 24, 2019 and will not appear on this ?Violation Notice?.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, facility did not ensure areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

Evidence: The following was observed:
1. One white crib had peeling paint along the railings of the crib that has been covered by tape. The tape was peeling back leaving brown jagged edges.
2. In the infant room, there was cable box cord hanging down within approximately 1' of the floor, within reach of children in the room. The cable box was not secured and could be pulled off the table approximately 2.5' high.
3. In the toddler room, there is peeling paint on the wall under the windows. Staff stated they usually have a border hanging over the area.

Description: Based on observation, facility did not ensure hazardous substances are kept in a locked place.

Evidence: Heavy Duty Non-Acid Washroom Cleaner/Disinfectant spray bottle was observed hanging from the sink in the unlocked kitchen.

Standard #: 22VAC40-185-340-F
Description: Based on observation, facility did not ensure children under ten years of age always are within actual sight and sound supervision of staff, except when child is using the restroom provided certain requirements are met.

Evidence: Between 10:20 am - 10:40 am, staff #6 was present with five children in the toddler room while staff #4 took a child into another room to change a diaper. Three children were sitting at a table that was up against the windows. Two children were sitting in high chairs behind the table. Staff #6 left the toddler room and went into the infant room to talk to staff leaving the five children in the toddler room without sight supervision by a staff. Staff #6 confirmed she did leave the children without sight supervision.

Standard #: 22VAC40-185-350-E-2
Description: Based on observation, facility did not ensure there is at least one staff to every five children when a child is of toddler age.

Evidence: Between 10:15 am and 10:30 am, staff #4 was present with six children in the toddler room. Staff #4 stated staff #1 had been helping in the room but staff #1 had gone to the other building for something. Staff #1 was observed returning and going into the kitchen to prepare food.

Description: Based on review, facility did not ensure an injury report had documentation of required information.

Evidence: The record for child #4 had an injury report completed on August 21, 2019 that did not have time of injury and time parent was notified.
