Comments:
An unannounced monitoring inspection was conducted from 9:45 am-12:15pm. At the time of entrance, 77 children were in care with 19 staff members present. Children were observed playing with table toys, reading in the library, engaged in group activities and outdoor play time. Infants were observed being held, fed, sleeping and playing with toys. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. . Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center inside was not maintained in a safe and operable condition.
Evidence:
1. An infant buggie had two torn seat covers.

Plan of Correction: I will order a new seat cover for the buggie.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the children's preschool bathroom, a safely lock under the diaper changing table was hanging on the handle of the cabinet and not locked. Two bottles of a cleaning product was inside the changing table cabinet.
2. In the Preschool 3 classroom, a safety lock was not placed in the locked position on a cabinet that contained a spray bottle of bleach water.

Plan of Correction: I will send a memo to the staff and speak to the teachers to remind them to keep them locked at all times.
