Comments:
An unannounced monitoring inspection was conducted on 09/24/2019 from 9:55 am - 11:50 am. At the time of the tour 78 preschool were observed with 11 staff. Children were observed during morning program time, outdoor play and lunch. Records were reviewed for eight children in care.

Violations were observed in four parts of the CDC standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center failed to maintain the name, address and telephone number of a person to be notified in an emergency which shall be kept at the center.

Evidence:
1. Emergency contact information was not on file for staff 1 who was present during the inspection.
2. Administrative staff confirmed that emergency contact information was not on file for staff 1.

Plan of Correction: Emergency contact information date sheets will be filled out as soon as staff arrive on site. Staff 1's sheet will be completed immediately.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that inside areas shall be maintained in a safe condition.

Evidence:
1. There is a broken area of wall tile in the bathroom of classroom A. The broken area of tile measures approximately five inches and is sharp in several areas.
a. This area of broken tile is accessible to children as it is located on an area of wall beside the children's toilet.

Plan of Correction: A work order has been placed, the broken tile will be covered until it can be replaced.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children.

Evidence:
1. The key to the storage closet in classroom D is stuck in the lock and can not be removed. This key is accessible to the preschool age children in care.
a. Two spray bottles of bleach and water were observed in this closet.
2. A bottle of bleach and a spray bottle of bleach and water was observed stored on top of the book shelf in classroom E.
a. The book case is approximately seven feet high.

Plan of Correction: A maintenance work order has been placed. The string will be removed and a door safety handle will be in place until the door knob can be replaced. ( 09/30/2019)

Staff will be retrained that all chemicals ( all keep out of reach of children items) will be locked and out of reach of all children at all times. (10/04/2019)

Standard #: 22VAC40-185-380-A
Description: Based on observation and interview, the center failed to ensure that there shall be a posted daily schedule that allows for flexibility as children's needs require.

Evidence:
1. There was no posted daily scheduled in classrooms C, D and F.
a. Classroom staff in these classrooms confirmed that the daily schedule was not posted.

Plan of Correction: A daily schedule will be posted by end of day 09/30/2019.

Standard #: 22VAC40-185-560-F
Description: Based on observation, the center failed to ensure that when meals are served shall indicate substituted food.

Evidence:
1. The substitution for lunch on 09/23/2019 was not noted on the posted menu.
2. Staff interviewed, stated that the lunch indicated for 09/24/2019 was served on 09/23/2019 however, this change was not noted.

Plan of Correction: Menu changes will be noted on menus. Kitchen staff will send out menu changes in advance.
