Comments:
A Subsidy Health and Safety inspection was conducted on September 24, 2019 to review supplemental health and safety requirements. This on-site visit was conducted from 2:00 p.m. through 5:00 p.m. There wetr sixty-seven children in the care of seven staff. The medication and medication paperwork was reviewed for one child. Eight children records and eleven staff records were reviewed and updated. Four board officer records were also reviewed.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Areas observed were: arrival, outside play, hand washing/bathroom procedures, snack, homework, and educational activities. Interactions between the children and staff were observed as appropriate and the children seemed comfortable with their daily routine and schedule.

Standard #: 22VAC40-665-540-A
Description: Based on record review, the center failed to obtain a tuberculosis screening for one staff within 30 days of employment.

Evidence: 1. Eighteen staff records were reviewed. 2. Staff #2 first day of hire was documented on 09/22/2018. 3. There was no documentation of obtaining a tuberculosis screening within thirty days of the first date of hire. 4. The director confirmed the tuberculosis screening was missing.

Plan of Correction: Staff #2 will immediately obtain a tuberculosis screening. Documentation will be sent to the licensing inspector.
