An unannoucned inspection was conducted today. The vendor was present with 1 child for a total of 3 points. The vendor has paperwork for the one child enrolled and present. The Vendor was given blank EPP and blank Attendance records. The child has a complete file and had toys available for use. Please call with questions 703-479-4679

Standard #: 22VAC40-665-160-B Description: Based on record review and interview, it was determined the vendor has not maintained a written hard copy of daily attendance for child enrolled and present. Evicwecne: 1, Aprils attendance only states it is for the month of April and does not list actual dates. 2. Vendor stated she did not have a sheet for May so she has not recorded it. Plan of Correction: I will record the date and time as it occurs.
Standard #: 22VAC40-665-180-2 Description: Based on record review, it was determined the vendor did not obtain copies of the CPS check and Sworn Disclosure Statement for both herself and a household member. Evidence: 1. Letter from Arlington County states the request for CPS check was sent to the wrong place. 2. Vendor stated she thought they had those copies. Plan of Correction: I will call the Arlington County Office to request copies of both Sworn Disclosure and CPS.
Standard #: 22VAC40-665-410-A-2 Description: Based on record review, it was determined the vendor did not practice two shelter in place drills in the last year. Plan of Correction: I will do two this year.
