Comments:
An unannounced monitoring inspection was completed on September 26, 2019 from approximately 12:45 p.m. to 3:15 p.m. One hundred and twenty-four children were in care and approximately nineteen staff were on premises during the inspection. Staff and children were observed in classrooms engaged in activities to include activities for infants, nap time, transitioning from nap time, and reading books. Interviews and interactions were conducted and medications were reviewed. Ten children?s records and five staff records were reviewed. The director and assistant director were available for the inspection and the director was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the outdoor space and interview, the licensee failed to ensure that areas and equipment of the center are maintained in a clean, safe and operable condition.
Evidence: 1. Observations on the playground included two metal round rods, approximately an inch in diameter, protruding approximately three inches above the ground, located approximately three inches beside two ends of a metal rocking boat. These were located on playground used by two year old children, causing hazards such as tripping into metal equipment, snagging, or entanglement. 2. In an interview, Administrator #1 advised that safety checks are completed on the playgrounds and acknowledged these hazards. Administrator reported an unknown amount of time in which the rods have been protruding from the ground.

Plan of Correction: The director called maintenance staff, who responded and corrected the hazard on site.

Standard #: 22VAC40-185-490-E
Description: Based on observation and interview, the license failed to ensure that any surface that has been contaminated with body fluids is cleaned and sanitized.
Evidence: 1. A restroom, adjoined to a school age classroom and used by school age children, was observed with four spots of dried bodily fluid smeared on the floor. School age children were observed arriving at the center to this classroom. 2. In an interview, Administrator #1, reported that kitchen staff is supposed to clean this bathroom daily before the school age children arrive at the center. Administrator #1 acknowledged that the staff did not clean and sanitize the contaminated surface.

Plan of Correction: The director called the responsible staff on site and directed her to clean the restroom. The director advised the program leader to not allow children to use this restroom until it was cleaned.
