Comments:

The licensing inspector conducted an unannounced monitoring inspection between 8:37 am until 11:03 am to determine the center's compliance with licensing standards. The licensing inspector observed eighteen children in care being supervised by four staff. During the inspection, the licensing inspector completed observations of the children in care completing a variety of activities to include participating in free play, playing with age appropriate equipment, diapering, bathroom breaks, and interacting with peers and staff. The center's designated classrooms, bathrooms, and kitchen areas were inspected by the licensing inspector. According to the center's staff, the playground area is currently not in use. The required posting were reviewed by the inspector. There were five children?s records and six staff's records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Medication is not being administered at this time. The center is currently providing transportation. Standard #:	22VAC40-185-160-C
Description:	Based on review of staff records and interview with staff, the center did not ensure staff resubmit TB test results every two years. Evidence: In Staff#1's (DOH: 6/12/17), Staff#2's (DOH: 5/27/17),Staff#3's (DOH: 7/27/17), Staff#4's (DOH: 7/27/17) record, there was no updated TB documentation found and the last dated TB results were 08/2017. Interview with staff confirmed there were no updated TB results for staff during the inspection.
Plan of Correction:	The center will ensure all staff TB are completed by 09/13/2019.
Standard #:	22VAC40-185-290-3
Description:	Based on observation at the center, the center did not ensure all electrical outlets have protective covers. Evidence: In the center's Threes/Fours classroom, there was an electrical outlet missing a protective cover.
Plan of Correction:	The center will ensure outlets are covered in classrooms.
Standard #:	22VAC40-185-350-E-1
Description:	Based on observation at the center and interview with staff, the center did not follow the required staff to children ratios for children from birth to the age of 16 months. Evidence: In the center's infant room, there was one staff present with five children in care during the inspection. Interview with staff confirmed that one staff was responsible for the care of the children in the infant room until staff returned from bus run.
Plan of Correction:	The center's director will make changes to staff schedule to adhere to the staff to children ratio.

Description:	Based on observation and interview with staff, the center did not ensure all cribs are identified for use by a specific child. Evidence: In the center's infant classroom, there was one crib observed unlabeled for a specific child. Interview with staff confirmed this crib is being used.
Plan of Correction:	The Director request staff corrected on site. Label have been added to crib.
Standard #:	22VAC40-185-500-A
Description:	Based on observations, the center did not ensure children's hands are washed with soap and running water after toileting. Evidence: In the center's infant room Staff#3 was observed diapering a child in care and the child's hands was not washed with soap and running water after diapering.
Plan of Correction:	The Director will speak to staff to remind them of proper handwashing procedures.
