Comments:
The licensing inspector conducted an unannounced monitoring inspection on Tuesday, October 1, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 09:40am and concluded at approximately 12:05pm. The center's director was present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 10 children in the direct care of 2 staff members. The director and one additional staff were on site during the inspection. Upon the inspector's arrival, the children were playing in different centers. The children were observed during free play. Some of the children were observed during teacher-led small group activities. Later, the children transitioned into a group activity. The staff read stories, sang songs, and the children followed along. Staff were observed having positive interactions with the children. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The required postings and required practice drills were observed and found to be in compliance. The first aid kit and non-emergency supplies were inspected. Five children's records and five staff records were reviewed. Per the director, the center does not administer medications at this time. The center has CPR, First Aid, Daily Health Observation, and MAT trained staff on site. The menu was posted and today's lunch consisted of ravioli, cheese, carrots, bread, bananas, and milk.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of five children's records and interview on 10/01/2019, the center did not obtain documentation that one child received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: 1) The immunization documentation in the record for Child #1, whose first day of attendance was 04/22/2019, was dated 9/03/2019. 2) During interview, a member of management reported the center did not have prior documentation of Child #1's immunizations.

Plan of Correction: Per the Center: Already corrected. In the future, the center will ensure immunizations are obtained before a child can attend the center.

Standard #: 22VAC40-185-140-A
Description: Based on a review of five children's records and interview on 10/01/2019, the center did not ensure that one child had a physical examination by or under the direction of a physician within one month after attendance.

Evidence: 1) The physical examination in the record for Child #1, whose date of attendance was 04/22/2019, was dated 09/03/2019. 2) During interview, a member of management reported the center did not have prior documentation of Child #1's physical examination.

Plan of Correction: Per the Center: Already corrected. In the future, the physical examination will be obtained within 2 weeks of enrollment and before the end of the first month of attendance for each child.

Standard #: 22VAC40-185-240-C
Description: Based on a review of five staff records and interview on 10/01/2019, the center did not ensure that three staff who work directly with children shall annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center.

Evidence: 1) The record for Staff #1, hired on 04/26/2010, contained a total of 7 hours of annual training from 2018-2019. 2) The record for Staff #2, hired on 09/02/2009, contained a total of 14 hours of annual training from 2018-2019. 3) The record for Staff #4, hired on 04/21/2010, contained a total of 6 hours of annual training from 2018-2019. 4) During interview, a member of management reported that Staff #1 completed over 16 hours of annual training, but the verification was not available for review at the time of the inspection.

Plan of Correction: Per the Center: The remaining hours for Staff #1 will be obtained and filed. Moving forward, staff will obtain the required hours of annual training.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview on 10/01/2019, the center did not ensure that disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence: 1) During the inspection, the licensing inspector observed the storage system used for disposable diapers had a lid on that had to be removed by hand. A disposable diaper was observed inside the trash can during the inspection. 2) During interview, a member of management reported the foot operated trash can was broken and not available for use at the time of the inspection.

Plan of Correction: Per the Center: A new foot operated trash can will be purchased and replaced today.
