Comments:
An unannounced monitoring inspection was conducted with center staff on September 24, 2019 to determine compliance with the licensing standards. There were 42 school age children present with five staff actively supervising. The children were having snack and working on homework before going outside to play. Snack served today: orange slices and Sun Chips. Two medications for one child were reviewed. Five children's files were reviewed and are in compliance. Three staff files were reviewed at the central office on 9/25/2019. The last fire inspection was conducted 10/7/2018. The last health inspection was conducted 3/14/2018. Evacuation drills have been completed monthly and the last shelter in place was conducted 4/30/2019. Time of today's inspection: 3:45 p.m. to 5:30 p.m.

Description: Based on review of two medications, the facility failed to have documentation of written parental authorization to administer medication. Evidence: there was no written parental authorization on file for Child A allowing the facility to administer two medications.

Plan of Correction: Corrected on site. The parent completed a written authorization to administer medication upon arrival at the center.
