Comments:
An unannounced monitoring inspection was conducted on 09/30/2019 from 11:10 am - 12:35 pm. At the time of entrance there were 16 children in care with four teaching staff and the program director. The children ranged in age from infant - three years. Children were observed during lunch and nap. Transition from lunch to nap was observed - which included hand washing and toileting.
Medication and emergency supplies were reviewed in addition to documented injury reports.
Records were reviewed for four children in care.
Violations were observed in two parts of the CDC standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-550-E
Description: Based on record review and interview, the center failed to maintain a record of the dates of practice shelter-in-place drills for one year.

Evidence:
1. The emergency log for the facility did not contain documentation of a second shelter-in-place practice drill for 2018.
2. The program director stated that two shelter drills had been conducted however, the drill implemented in March of 2018 had not been documented on the center emergency log.

Plan of Correction: Going forward all emergency practice drills will be documented on the emergency log for the facility.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home the food container shall be clearly dated and labeled in a way that identifies the owner.

Evidence:
1. One can of powdered infant formula, that had been opened, was not dated and labeled with the child's name.
a. Staff in the infant room verified that this can of formula was in use for a child in care.

Plan of Correction: The can of formula was dated and labeled with the child's name during the inspection.
