Comments:
The licensing inspector conducted an unannounced complaint inspection on September 24, 2019 in response to a complaint received by our department on September 11, 2019. The inspector was on-site from approximately 10:15 AM to 11:30 AM. The allegations of the complaint were related to inadequate staff-to-children ratios, supervision, and infant sleeping requirements. Observations were made, interviews were conducted, and records were reviewed. The preponderance of the evidence gathered during the investigation does not support the allegations; therefore; the complaint is determined not valid. Violations were found in areas that were not a part of the original complaint. See the violation notice for more detail.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on observation, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. Evidence: 1. The licensing inspector observed the playground equipment described as an airplane seesaw in use by the three and four year old classrooms during the inspection. The seesaw seats reach a height of 3 foot. The seesaw seat on one side is located a distance of 2 foot, 1 inch from a wooden retaining wall, which is 9 inches in height. The rear seating area of the airplane seesaw reaches a height of 19 inches. The rear of the equipment is located a distance of 2 foot, 7 inches from a retaining wall, which is 1 foot, 1 inch in height. 2. The licensing inspector observed that four measurements of the mulch surrounding the airplane seesaw had a depth of 4 inches and one mulch depth measurement was 3 inches. This playground equipment requires a fall zone free of hazardous obstacles and resilient surfacing that complies with minimum safety standards.

Plan of Correction: Per administration "Children will not use play equipment until fall zone is free of hazardous obstacles. If issues can't be fixed the pay equipment will be thrown away."
