Comments:
The licensing inspector conducted an unannounced monitoring inspection on Monday, September 16, 2019 between 2:00 pm to 4:17 pm to determine the center's compliance with licensing standards. The children in care were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 55 children and 18 staff present including administrative staff. The licensing inspector completed observations of the children participating in a variety of activities to include, sleeping in their cribs, feedings by staff, engage in play with age appropriate equipment, interacting with peers and staff, participating in circle time, and eating snack. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were reviewed by the inspector. During this inspection, there were six children's records and five staff's records reviewed by the inspector. Standard #: 22VAC40-185-60-A
Description: Based on review of children's record and interview with staff, the center did not ensure records contain all information.

Evidence: In Child#6's record (start date: 9/16/19) the first designated contact document was a parent's, name, address, and phone number and the second designated contact documented address was incomplete.

Plan of Correction: The address was filled out by the parent on pick up.

Standard #: 22VAC40-185-280-B
Description: Based on observations, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: The following observations were made of unlocked hazardous substances with the labels"keep out of reach of children" "warning" throughout the center:
1. In the Twos classroom, there was three containers of disinfectant wipes located on a the top of a cabinet.
2. In the Young Toddlers classroom, there was hand sanitizer hanging on the wall on a hook.
3. The center's kitchen door was observed opened and there was an unlocked cabinet under the sink containing multiple disinfectant cleaners which is accessible to the children.

Plan of Correction: Staff were reminded to keep these and all other hazardous substances locked up.

All of the identified items were locked up.

The kitchen cabinet was locked.
