Comments:
An unannounced monitoring inspection was conducted between 8:00am and 11:00am. At the time of the tour there were 29 children in care with 8 staff present. A sample of 3 children and 2 staff records were reviewed. The director confirmed there were no children with diagnosed food allergies at the time of inspection. Children were observed during circle time, being read a story and during diapering. Infants were observed sleeping in cribs and on the floor with staff.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a record review and interview the center did not ensure there was an annual fire inspection report.
Evidence:
The last documented fire inspection was dated 04/12/18. The Director confirmed that was the last inspection at the center.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The office manager called the fire marshal's office during the inspection and left a message to schedule an inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement of the surfacing, the center did not ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 .
Evidence:
There was about 4 inches of resilient surfacing in the fall zones of the 2 swing sets and toddler composite structure. Areas of the fall zone had grass patches.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The board has approved funding for mulch and a message has already been left for a company to come out and deliver. Once it is delivered, the director will send a copy of the delivery invoice to the inspector.

Standard #: 22VAC40-185-430-C
Description: Based on observation the center did not ensure play equipment met all requirements.
Evidence:
The playground swing set had 8 S Hooks that were opened more than the thickness of a penny. The Director was shown the s hooks and confirmed they were open.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The director will contact maintenance to tighten all shooks on the swings.

Standard #: 22VAC40-185-500-B
Description: Based on observation the center did not ensure all requirements were met when diapering.
Evidence:
A staff member was observed changing a child's soiled diaper. After removing the soiled diaper from the child, the staff used her hand to lift the lid of the foot operated pail and disposed of the diaper. After the diaper change, the staff member did not clean and sanitize the diapering surface.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The director also observed the staff diapering and confirmed diapering requirements were not met. The director will retrain staff on proper diapering and will monitor for compliance.
