An unannounced inspection was completed on 10/08/2019 from 9:15 A.M.-1:30 P.M. At the time of the inspection 16 children were in care with three staff and one administrator present. Five children records, four staff records, six injury reports, evacuations drills, first aid kits, emergency supplies, asbestos management plan, asbestos inspection record, health inspection, fire inspection, playground, and posted required information were reviewed. Children were observed outdoor play, behavioral guidance, story time, handwashing, lunchtime, diapering, and transition to naptime. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-60-A Description: Based on review of children records, the center failed to ensure that all children had a complete record on file with all required information.
1. Child #4 did not have a registration form on file and was missing all required information. Date of enrollment for Child #4 was 08/19/2019.
2. Administration verified that the file did not contain a registration form, and that Child #4 began enrollment at the center on 08/19/2019.
Plan of Correction: Administration looked thru other children files to see if it had gotten mixed in with another file. There were several children that started on the same day. The form could not be located. Administration will contact the parent today and request a new registration form be completed and turned in before the next day of attendance.
Standard #: 22VAC40-185-90--A Description: Based on review of children files, the center failed to ensure that a written agreement between the parent and the center was in a child's record by the first day of attendance.
1. Child #4's file did not contain a written agreement between the center regarding a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease as defined by the State Board of Health.
2. Staff verified that Child #4's file was missing the required written agreement between the center and the parent.
Plan of Correction: Administration will contact the parent today and request a copy of the agreements be signed and dated before the child is attendance again.
