Comments:
An unannounced monitoring inspection was conducted on October 3, 2019 to determine the centers compliance with licensing standards. Staff Records were reviewed at the Downtown administrative office on September 25, 2019. The on site inspection was initiated at 3:00 pm and concluded at 4:10 pm. Upon the inspectors arrival there were thirty school age children with two staff in the school gym. Children were playing a game of four corner dodge ball. Four additional children and one staff were observed in the cafeteria working on homework. Later all children gathered in the cafeteria for snack. Hand washing was conducted and tables cleaned. Children also participated in a team building activity using straws and tape to build a structure. Dismissal time was also observed with staff greeting parents and sign out sheets being completed. Today's snack consisted of pears and pretzels. Systems were in place to ensure supervision during rest-room breaks and hand washing. Staff interacted with children in a positive manner and circulated around the gym participating in activities. All areas used for child care were inspected. The facility has age appropriate materials and supplies. The facility had the following required information posted: license, last inspection notice, emergency numbers, menu, daily schedule, evacuation map/ procedures, and various parent information. The first aid kit and non-medical emergency supplies were inspected. The inspector interacted with staff and children in care.

Five children's records were reviewed along with emergency information for three staff.
Three staff records were reviewed on September 20, 2018.
Medications were reviewed.Staff on site have current CPR, First aid, daily health observation and MAT.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of staff emergency contact information, the center did not main the name, address and phone number of a person to be notified in an emergency for each staff which is kept at the center.

Evidence:

Staff #3, who was observed working on October 3, 2019, did not have the name, address and phone number of a person to be notified in an emergency that was kept on site.

Plan of Correction: The director had the staff complete the missing information while the inspector was on site. Per the director: I will have staff complete their emergency contact information at the time of hire.

Standard #: 22VAC40-185-550-D
Description: Based on review of the emergency evacuation drill log, the center did not implement a monthly practice evacuation drill.

Evidence:

1. The evacuation drill log did not contain documentation of a practice evacuation drill for the month of September 2019.
2. The director reported that an evacuation drill had not been conducted during the month of September 2019.

Plan of Correction: Per the director: I will add a calendar alert to remind us to conduct a drill monthly.
