Technical Assistance:
The required retention of children's and staff records (22VAC 40-185-50.B) for two years after termination of services or separation from employment was discussed.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 11:44 AM and departed at 2:50 PM. One hundred four children were in care with nineteen staff. All classrooms were observed. Lunch service, outdoor play and nap time were observed. A sample size of twelve staff records were reviewed. Medication administration and injury records were reviewed.

An exit meeting was conducted with the owner and assistant director prior to closure of the inspection.

Standard #: 22VAC40-185-270-A
Description: Based upon observation, the facility has not ensured that areas and equipment, inside and outside, are maintained to be clean and safe.
Evidence:
There is a metal cover on the toddler playground where erosion has exposed a hole that is four inches deep along one corner, creating a potential tripping hazard where a child call fall and be injured on the metal corner.

Standard #: 22VAC40-185-280-B
Description: Based upon observation and staff interview, the facility has not ensured that potentially hazardous substances are kept in locked locations using a safe locking method that prevents access by children.
Evidence:
1. The safety clasp on the cabinet under the changing table in the older infant classroom was not connected. There was a spray bottle of sanitizing fluid in the cabinet.
2. Staff 14 verified that she had tended to an upset child and failed to reattach the closure.
3. During the facility tour, the door to the staff lounge was not locked. The safety clasp on the cabinet under the sink in the lounge was not connected. Various potentially hazardous cleaning products were in this cabinet.
4. Staff 1 verified that the lounge door was not locked and the safety clasp on the cabinet was not connected.

Description: Based upon review of injury records and staff interview, the facility has not ensured that parents are notified by the end of the same day of children's minor injuries.
Evidence:
1. An injury record documented that child 1 experienced a minor injury on 6/26/2019. The same injury record documented that the parent was not notified of the injury until 6/27/2019 at 7:21 AM.
2. Staff 1 verified the dates and documentation on the injury record.

Description: Based upon review of children's injury records, the facility has not ensured that the record of children's injuries includes documentation of the date and time the parent was notified of the child's injury.
Evidence:
1. An injury record dated 7/10/2019 for child 2, although signed by the parent, does not have documented the date and time the parent was notified of the injury.
2. Staff 1 veri fied that the date and time of parent notification was not documented on the above injury record.
