63.2(17) License & Registration Procedures

Comments:
An unannounced renewal study was conducted from 8:45 a.m. - 11:00 a.m. There were 24 children in care being supervised by 3 staff members. The children were observed participating in structured center play, preparing for and eating snack. Snack served today is wheat crackers, carrots, and water. Six children's records were reviewed and in compliance. Five staff records and three agent records were reviewed. There are reportedly no medications being administered to children at this time. The most recent emergency escape drill was conducted 09/11/2019 and the most recent shelter in place drill was conducted 10/16/2019. Standard #: 22VAC40-185-280-B
Description: Based on observation it was determined that not all harmful substances were kept in a locked area inaccessible to children. Evidence: 1. A bottle of cleaner, disinfectant wipes, and aerosol disinfectant spray was observed in an unlocked closet accessible to children in the Pre-school classroom. 2. A bottle of sanitizer and a bottle of cleaner was observed in an unlocked closet in the Montessori room.

Plan of Correction: The hazardous substances were made inaccessible during the inspection.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on review of three agent records it was determined that not all records contained results of a central registry check within the first thirty days of becoming an agent or a sworn statement or affirmation on file. Evidence: Agent 1 (start date: 08/29/2016) did not have documentation of a central registry check on file. Agent 1 (start date: 08/29/2016) did not have documentation of a sworn statement on file.

Plan of Correction: An up to date central registry check will be sent to administration of an additional central registry check will be completed. Agent 1 will complete a sworn statement and submit the results to administration.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff records it was determined that not all staff records contained documentation of a central registry check within the first 30 days of employment. Evidence: Staff A (start date: 02/13/2019) and staff B (start date: 07/25/2018) did not have documentation of a central registry check on file.

Plan of Correction: The central registry checks will be sent to administration or a new central registry checks will be conducted for staff A and staff B.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on review of three agent records it was determined that not all records contained results of an eligible determination letter based on fingerprinting by September 30, 2018. Evidence: Agent 1 did not have documentation of an eligible determination letter based on fingerprinting on file.

Plan of Correction: Agent 1 will make an appointment to obtain a fingerprint check and results will be submitted to administration.
