Comments:
An unannounced monitoring inspection was conducted today beginning at 12:00pm and ending at 4:00pm. There were 89 children (ages 4 months to 4-years-old) in care being supervised by 16 staff. Children were observed eating lunch, preparing for nap, napping, and playing in Centers. Lunch today was corn dog nuggets, baked beans, fruit, and milk. Eight children's files and eight staff files were reviewed. Medication and logs were reviewed. The emergency drill log was reviewed and in compliance. The First Aid kit, flashlight, and radio were reviewed and in compliance.

Violations:
Standard #: 22VAC40-185-80-A
Description: Based on review of documentation and staff interview. the Facility did not maintain an accurate written record of children in attendance.
Evidence: In Classrooms 2.4.and 6 the written attendance did not match the number of children present during the inspection.

Plan of Correction: We will emphasize making sure daily records match on paper and on the Tadpoles.

Standard #: 22VAC40-185-240-A
Description: Based on review of staff records and staff interview, the facility does not have a staff member who is trained to give the Auvi-Q medications.
Evidence: Child 4 has an Auvi-Q medication for food allergies but there is no documentation of staff having the training to give the medication.

Plan of Correction: We will have someone complete the training today and then will have more staff take the training as well.

Standard #: 22VAC40-185-270-A
Description: Based on observation the Facility did not maintain all outdoor areas in a safe condition.
Evidence: On the Toddler and School age playgrounds there were multiple areas where the black weed stop under the mulch was sticking up through the mulch posing a tripping hazard.

Plan of Correction: We will add more mulch to the playground.

Standard #: 22VAC40-665-670-G
Description: Based on review of written documentation and staff interview, tummy time is not being documented daily.
Evidence: There was no written documentation of tummy time during the inspection and the staff reported that they had not documented it yesterday or today.

Plan of Correction: We have a form to document tummy time so we will monitor for the next week to make sure they are doing the paperwork.
