A Subsidy Health and Safety inspection was conducted 10/14/19 from 3:15pm - 4:15pm. During the inspection there were 59 school age children in care with four staff. Children were observed participating in various activities, eating snack, and doing homework. Records were reviewed for six children while at the facility. The records for four staff were reviewed at the central office on 10/16/19. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-500-F Description: Based on interviews and observation, it was determined that the vendor did not ensure to post , with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions.
Evidence:
1. There was not a current list of all children?s allergies, sensitivities, and dietary restrictions posted anywhere in the facility.
2. Staff #4 confirmed that there was not a list that included all of the children?s allergies, sensitivities, and dietary restrictions available for viewing during the inspection.Plan of Correction: The vendor responded: A new list with all of the children?s allergies, sensitivities, and dietary restrictions will be created and kept at the facility that can be accessed by staff.
