Comments:
A Subsidy Health and Safety inspection was conducted on 10/8/19 from 2:20pm - 3:30pm. At the time of entrance there were 38 children ages four years old to ten years old in care with four staff. Children were observed participating in playing in the gym. Records were reviewed for five children while at the facility. The records for four staff were reviewed on 10/9/19 at the central office. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-500-F
Description: Based on interviews and observation, it was determined that the vendor did not ensure to post , with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions.

Evidence:
1. There was not a current list of all children?s allergies, sensitivities, and dietary restrictions posted anywhere in the facility.
2. Staff #3 confirmed that there was not a list that included all of the children?s allergies, sensitivities, and dietary restrictions available for viewing during the inspection.

Plan of Correction: The vendor responded: A new list with all of the children?s allergies, sensitivities, and dietary restrictions will be created and kept at the facility that can be accessed by staff.

Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records, it was determined that the vendor did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:
1. The record for staff #1 (date of hire 10/8/19) did not contain documentation of a TB screening.
2. Staff #5 confirmed that staff #1 had not completed a TB screening at the time of employment.

Plan of Correction: The vendor responded: Staff #1 will be sent to complete a TB screening. Going forward we will ensure that all new staff had completed a TB by the time of employment, and that it has been completed within the last 30 days prior to employment.
