Comments:
An unannounced monitoring inspection was conducted today from 10:30am-12:00pm. There were 38 children (3-6yrs.) supervised by 6 direct care staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing with blocks, cleaning, playing outdoors, and engaging freely. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, 3 of 4 staff records did not contain documentation of all required information.

Evidence:
1. Staff A's (date of hire: 7/1/2019) record did not contain documentation of emergency contact information and proof of education.
2. Staff C's (date of hire: 7/1/2019) record did not contain documentation of proof of education.
3. Staff D's (date of hire: 7/1/2019) record did not contain documentation of an employment application or resume, and proof of education.

Plan of Correction: I will obtain all missing information by October 30, 2019.

Standard #: 22VAC40-185-280-B
Description: Based on classroom observation, 2 of 3 classrooms did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the Bluebirds classroom (children ages 3 years old), there was a container of disinfecting wipes and two other chemical containers in the children's restroom.
2. In the Oak Tree classroom (children ages 4-6 years old), there was a container of multipurpose cleaner and disinfecting wipes in the bottom of an unlocked teacher cabinet.

Plan of Correction: Chemical items will be labeled and locked away.

Standard #: 22VAC40-185-510-F
Description: Based on a review of children's medications, 1 of 3 medications was not in the original container with the direction label attached.

Evidence: Child #5's (5 years old) Children Allergy Relief was not in the original box.

Plan of Correction: When it expires, we will get a new one with the box.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 3 of 7 injury reports did not include documentation of all required information.

Evidence: Three injury reports did not include documentation of the time of parent notification.

Plan of Correction: I will remind teachers to fill forms out completely.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 of 4 staff records did not contain documentation of a central registry finding within 30 days of employment.

Evidence: Staff D's (date of hire: 7/1/2019) record did not contain documentation of a central registry request or findings.

Plan of Correction: I will mail it out on October 24, 2019.
