An unannounced monitoring inspection was conducted on October 15, 2019 from approximately 10:00 am to 1:00 pm. Six staff were present with thirty-five children. Five children's records were reviewed.

Standard #: 22VAC40-185-260-A Description: Based on review, facility did not ensure there is an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence: The most recent report from the appropriate fire official having jurisdiction is dated August 30, 2018 which is more than a year old.Plan of Correction: An inspection was requested during this licensing inspection.
Standard #: 22VAC40-185-260-B Description: Based on review, the facility did not ensure there is an annual health inspection conducted.

Standard #: 22VAC40-185-330-B Description: Based on observation, facility did not ensure where playground equipment is provided that resilient surfacing shall comply with the minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence: Measurements obtained confirmed the platform for two slides on the large composite of play equipment are 5' 2" high and fall zone is 7' 1" at the end of the slide chutes. Based on the height of the slide, the required fall zone would be 9' 2" at the end of the slide chute (height of slide + 4').Plan of Correction: The Improvement Association will request an Allowable Variance.
