Licensing Inspector arrived at 6:30 and departed at 7:30 AM to conduct a site inspection. At the conclusion of the inspection there were 11 school-aged children in care with two staff. Children were observed participating in technology activities utilizing tablets and playing with table top toys. Medication and center documents were reviewed. The sample size was five staff records.
Licensing Inspector arrived at the AlphaBEST office at 12:30 and departed at 2:00 PM to review staff records. The sample size was three staff records.

Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, it was determined that the facility did not ensure that all required elements were present in each child's record.
Evidence:
The record for child #3 did not include an address for one of the two emergency contact persons.Plan of Correction: Site director will contact parent to obtain address and place it in the record.
Standard #: 22VAC40-185-510-G Description: Based on review of the medication at the facility it was determined that the facility did not ensure that medication dispensed from a pharmacy is maintained in original, labeled container.
Evidence:
There was Auvi-Q in the medication box that was not in its labeled container.Plan of Correction: Site director will contact parent to obtain pharmacy label.
