Comments:
An unannounced monitoring inspection was conducted on 10/17/2019 from 3:00 pm - 4:50 pm. At the time of entrance there were 66 school age children in care with six staff. Children were observed during afternoon program time which included outdoor play, large motor games in the gym, table games in the cafeteria and snack.
Records were reviewed for nine children in care. Two medications were reviewed.
Violations were observed in two parts of the CDC standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.

Evidence:
1. The record for child 1 lacked documentation of a work phone number for each parent.
2. The record for child 2 lacked documentation of a second emergency contact.

Plan of Correction: Parents have been notified and have updated missing information.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to ensure that tables used for snakes shall be sanitized after each use for feeding.
"Sanitized" is defined as allowing the surface to air dry after application of the sanitizing solution.

Evidence:
The inspector observed staff 1 spray sanitizing solution on two tables that had been used for afternoon snack. Staff 1 immediately wiped each table after application of the sanitizing solution rather than allowing the tables to air dry as required.

Plan of Correction: Staff had previously been instructed to follow directions on sanitizer, which says you can wipe or air dry, but will only air dry going forward.
